For teenagers

If you are a person under the age of 18, the consent of your parent or legal guardian* for your treatment is necessary. If you are a person between 16 and 18 years of age, both your consent and the consent of your parent or legal guardian are required.

If you are a person over 15 years of age and want to have medical consultation or prophylaxis examinations at the FEDERA Health Center, you have two options:

  • come with your parent or legal guardian, and in the absence of such, with your de facto guardian**;
  • • bring to the appointment their written consent to provide you with the above-mentioned services (a sample consent can be found below).


If you are a person under 15 years of age and want to have a consultation or examination, it is necessary for your parent or legal guardian to be physically present with you, and if that is not possible, you need to be assisted by your de facto guardian.

In the case of treatments, the physical presence of your parent or legal guardian is always necessary.

If the parent, legal guardian, or de facto guardian does not consent to medical consultations, examinations, or treatments, the decision can be made by the guardianship court. The FEDERA Health Center may apply to the guardianship court for such permission.

*legal guardian – a person other than a biological parent who – based on the official decision (i.e. made by a court) – has the right and obligation to take care of you as a custodian/parent.

**de facto guardian – a person who takes care of you on a daily basis, although they are not formally obliged to do so
